If you wish to be reimbursed for our sessions using out-of-network (OON) benefits, please contact your insurance company directly and ask them the following questions:

1. I'm hoping to submit a Superbill for OON outpatient mental health services and I will be paying them directly. What are my out-of-network benefits? Specifically, how much of the session cost will I be reimbursed through my plan?

2. If I do have OON benefits for this service, does my deductible apply?

3. The therapist will give me a Superbill (receipt) for my appointments; do you have a specific form I need to complete in addition to the Superbill in order to get reimbursed? Where do I send this form(s)?

4. How will I receive payment?
